Full Job Description

The successful candidate will work in a strength-based way and aim to make a real difference to the lives of Adults with Mental Health and/or Learning Disabilities and their carer's. There may be a requirement at times to support work with individuals who have a physical disability. Support to develop skills in these areas will be provided.

You will work in accordance with the following legislation: The Care Act, Mental Health Act & Mental Capacity Act. Good Communication, time management and IT skills are essential. The team has a long history of multi-disciplinary and partnership working with other agencies. You will also undertake Adult Safeguarding investigations, focus on facilitating complex hospital discharges from the acute mental health wards, undertake statutory responsibilities for social circumstances reports and attendance at mental health tribunals. You will also take an active role in Decision Support Tool Meetings and work with individuals to maintain independence.

This is a rewarding service area within social work and the team offers good support within a friendly atmosphere. You will receive regular supervision and have access to appropriate training and opportunities for personal development.

The Gedling living well Team are looking for a full time and part time enthusiastic, highly motivated, and qualified Social Worker, working with individuals who have Mental Health and/or a Learning Disability. This is an exciting opportunity to become part of a specialist team offering assessment and commissioning support to these individuals.
