Full Job Description

An exciting opportunity has arisen for a Health and Housing Options Advisor to work across both the Acute and Community hospitals in North Northamptonshire.

Our successful Health and Housing Partnership, involving the Council, Kettering General Hospital and Northamptonshire Healthcare NHS Foundation Trust, has been in place since 2017. The partnership focuses on integrating health and housing locally, by making more effective use of resources and ultimately improving the health and wellbeing of residents locally.

What will you be doing?

The main aim of the role is to help prevent delays in hospital discharge when there is a housing issue by finding and assisting patients with suitable housing solutions where possible and being the key point of contact for hospital colleagues.

The role will involve building relationships and knowledge across the sectors and working closely with health and housing teams across North Northamptonshire to support people with complex needs to remain in their own home or to help them find an alternative housing solution. In addition, there will also be the opportunity to be involved in the delivery of preventative solutions through project work to help address the identified gaps in health and housing provision locally.

You will have experience of working within a local authority or equivalent organisation along with experience of working with or understanding of vulnerable people or those with complex needs.

You will have a proven ability to work effectively with a variety of teams and structures, be able to work well under pressure and use your initiative to manage and prioritise your workload.

To succeed in the role, you will have well developed negotiating and problem-solving skills in addition to excellent communication skills to provide advice and assistance to achieve positive outcomes.
