Full UK manual driving licence is essential
Are you passionate and enthusiastic with great people skills? Do you love helping people in need? If so and you could also make someone feel cared for and comfortable, this role could be for you!
You will support Derriford Hospital by providing transport, using a small British Red Cross vehicle, for a safe discharge from the hospital for people within Plymouth area. You will be a friendly face to someone who is being discharged and may require assistance with immediate resettlement at home and in the community.
What will a day in the life of a Support Worker involve?

  • Providing practical and emotional support and guidance to service users from the point of discharge and for up to 72 hours afterwards.

  • Responding to referrals from Clinicians and Community Health Professionals

  • Assessing service users' needs, completing and following a support plan for them.

  • Supporting service users in the immediate days after hospital discharge to prevent readmission.

    A good knowledge of services provided by the NHS and Social Care.

  • Being able to support and assess vulnerable peoples needs.

  • Dealing with issues in a diplomatic and confidential manner.

    £21,840 a year - Fixed term contract, Holidays: 36 days annual leave (including bank holidays) + option to buy 5 extra days.
