Full Job Description

A fantastic opportunity has arisen to work within the brand new Sister Winifred Laver Centre as part of Gateshead Health NHS Foundation Trust. This post involves working in partnership with Gateshead Local Authority and a large multidisciplinary team.

The Sister Winifred Laver Centre is a sixty bed intermediate care facility for patients recovering from an illness or injury. The centre is for patients requiring a period of recovery, rehabilitation or reablement which does not need to be provided in hospital but can't be provided in their home due to their support needs.

The centre has two main aims, hospital avoidance in the first instance as well as supporting people who have had a stay in hospital but are currently unable to be supported at home. The centre provides integrated care from both Gateshead Local Authority and Gateshead Health NHS Foundation Trust.

The aim is to provide a multidisciplinary assessment and treatment plan to maximise independence and health and wellbeing. As well as providing timely discharge from hospital or preventing unnecessary hospital or residential care admissions., The post holder will work effectively within a multi-disciplinary/agency team attending to patients identified as requiring intervention to prevent their health and wellbeing deteriorating and to promote and encourage rehabilitation. In addition, there will be planned activity to provide hands on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care and to maximise patient independence/promote rehabilitation to allow the patient to return to their own home.

This post holder will be required to ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty.The post holder will support the senior team in the safe and effective delivery of patient care.

This post will contribute to the safe delivery of effective rehabilitation working flexibly over 7 days per week.
