Full Job Description

  • To be involved in the reviewing and updating of resident's care plans on a monthly basis

  • To ensure any change of needs is reflected in the resident's care plans to ensure all physical and emotional needs are being met and carried out by the care team.

  • To ensure all monthly risk assessments such as Water low tool, falls risk tool and MUST chart is completed accordingly

  • To be responsible for the safe administration of medication to our residents, to follow the homes medication policy and procedures.

  • To ensure all body maps are being completed and reviewed on shower/bath days

  • To ensure all residents written risk assessments are reviewed monthly and updated accordingly

  • To take part in annual resident care plan reviews, organising a meeting with the resident or POA

  • To work alongside the Registered Manager in care plan monthly audits and implement necessary changes accordingly

  • You will look after the different needs of our residents to enable us to deliver quality, person centred care and support.

  • Liaising with outside professionals to ensure residents live well

  • Monitoring and driving standards of care

  • To attend monthly senior team meetings to assess the care services using CQC KLOEs

  • Your role will involve supervising and inspiring a team of dedicated care assistants alongside the management team

  • Ensuring residents are protected and safeguarded against harm and abuse at all times.

  • Excellent communication and customer skills, at all levels

  • NVQ 3 or equivalent

  • Excellent IT skills and knowledge with using Microsoft Word & Excel

  • IT application usage experience to support electronic Care Planning System implementation

  • Need to understand the KLOEs

    Pay: £12.30-£13.50 per hour

    Expected hours: 24 - 36 per week
