Full Job Description

The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within the Clinical Hubs delivering care on behalf of the Primary Care Networks (PCNs) outside of core general practice hours. Our clinical hubs are based in Redcar, Middlesbrough, Brotton and Garth.

The Nursing Associate will work in close partnership with the other key members of the Hub Service, ELM Alliance Management Team and external organisations to implement & deliver the Services on time and within budget.

Work with the qualified nurses to deliver individual treatment packages with an emphasis on collaborative care that focuses on recovery and the need to work towards discharge where appropriate. The post holder will work with other members of the Multi- disciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient.

South Tees area comprises of 46 general practices with a long history of working together in their respective localities of Langbaurgh, Eston and Middlesbrough. It covers a total population of 300,000 and includes some of the most deprived areas in the country. The area has traditionally been one of the highest levels of emergency and unplanned attendance and hospital admissions in the country., ELM Alliance LTD (Eston, Langbaugh, Middlesbrough Alliance) is a GP Federation, established in 2016; wholly owned by the local GP Practices in Middlesbrough, Redcar and Cleveland. The organisation delivers primary care and community services by contract with the NHS CCG, Local Primary Care Networks and Local Authorities. In addition, we provide enablement and support to our members.

ELM was rated GOOD by the Care Quality Commission in the latest review in 2019.
