Full Job Description

This is a new role and new team within Brent HTT. Outreach team is a sub-team of HTT, with a focus on supporting residents of NW10 and NW2, who frequently present to Accident and Emergency in various hospitals. This role will entail our outreach staff working with the GPs, Psych Liaison, A&E, and other third party sectors in and around London., To support members of the Home Treatment Team in ensuring a high standard of care to service users in acute crisis. To participate in the care planning, provision and evaluation of patients' needs under the supervision of trained staff.

To assist in the provision of a 24-hour / 7 day a week crisis service to the residents of the relevant borough. The Post holder is however not required to do night shifts., As a Band 4 Support Worker, you will be working with the HARROW Acute Home Treatment Team, where excellent communication skills are essential in gaining a clear understanding of your patients' particular issues, building confidential and constructive relationships whilst maintaining boundaries.

The post holder will contribute to the key functions of the clinical area by carrying out assigned tasks, involving direct and indirect care to patients, under minimal supervision and as directed by the HTT shift co-ordinator and/or Team Manager. Following personal development planning and targeted training, the competent practitioner will be in a position to work more autonomously in supporting patients., This role presents an excellent opportunity to work in a challenging, fast paced, and diverse role which requires you to be confident in your decision making and positive risk taking. You will be working closely with your patients and their family, supporting them in finding a best way forward to meet their needs. You will be provided with training and be supported by teams with experienced staff. You will also play a very important role in helping us transform the way CNWL deliver urgent and acute care.

Opportunity for Band 4 positions has arisen in the Brent Home Treatment Team within the Urgent Care Service. To apply, you will have to have experience of working as part of a team with people suffering with acute/severe and enduring mental illness.

HTT provides a 7 day service to adults with a serious mental illness in an acute crisis who would, without intervention from the service, require hospitalisation. The idea being that by offering intensive home treatment to patients, formulating crisis plan and utilizing their support network, it can reduce the chance of them escalating to crisis point, the risks of deterioration and harm to self and others. You will act as a key worker in providing alternative to inpatient admission including utilising alternative community provisions such as the Coves, Hestia, (crisis alternative) and other community services.
