We have an exciting opportunity for a compassionate and dedicated Assertive Alcohol Outreach Worker to join our dynamic team. The aim of this innovative approach is to provide intensive support to individuals struggling with alcohol dependence in community settings, with the goal of reducing A&E admissions and improving overall health outcomes.
This is an excellent opportunity for someone with extensive experience of Alcohol misuse issues looking to take the next step in their career with a charity that will support your continued personal and professional development.

Hours: 37.5,

  • Engage with individuals referred from hospital teams, particularly those with frequent admissions or complex needs related to alcohol dependence.

  • Provide intensive support through regular home and community visits, utilizing a patient-led approach to address health and social care needs.

  • Collaborate with service users to develop care plans tailored to their needs, focusing on harm reduction and improving quality of life.

  • Work closely with hospital liaison teams and key partners to facilitate referrals and coordinate care pathways.

  • Maintain accurate and detailed client interactions and progress records, ensuring compliance with relevant regulations and guidance.

  • Participate in multidisciplinary meetings to discuss holistic support needs and coordinate services effectively.

    Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

    The post is contracted until March 2025, with a strong possibility of continuing beyond that.
